I understand that my employment and continued employment is contingent upon satisfactory consumer reports and criminal background checks. I authorize the Company to investigate, verify and discuss my past employment, education, training and references and all information set forth in my employment application, by contacting my prior employers, colleagues, educational institutions, state agencies and other references. I authorize any entity or person named in this application to provide the Company with any and all information in their possession, custody or control regarding me. I hereby indemnify, release, forever discharge and agree to hold American Restoration & Maintenance Corp. dba Performance Industrial, its agents and all persons, companies and corporations supplying this information harmless from any and all claims, demands, judgments and legal fees arising out of or in connection with this investigation, the results, or any lawful use of the results or disclosure thereto.
I understand that if a job offer is extended a physical examination may be required to verify fitness to work. Any such examination will occur after a job offer has been extended but prior to beginning work. Offers of employment are contingent upon successfully completing all pre-employment exams and/or testing. I also understand that if the results of the examination/testing indicate that I cannot safely perform the essential functions of the position for which I have applied, with or without a reasonable accommodation, the conditional offer will be revoked and I will no longer be considered a qualified candidate for employment.
I understand the physician or other licensed professional designated by the Company to conduct the pre-employment physical examination is not my health care provider. I also understand that the pre-employment assessment is not a comprehensive medical examination or a health care service for my benefit, nor does it constitute establishment of a provider-patient relationship.
I understand that in accordance with the Genetic Information Nondiscrimination Act (GINA), the Company asks that I not provide any genetic information as part of pre-employment screening. "Genetic Information" as defined by GINA includes an individual's family medical history, the results of an individual's or family member's genetic tests, the fact that an individual or an individual's family member sought or received genetic services, and genetic information of a fetus carried by an individual or an individual's family member or an embryo lawfully held by an individual or family member receiving assistive reproductive services.
I also consent to the release to the Company all records related to the physical examination that contain relevant information about my fitness and ability to perform the essential functions of the position for which I have applied. I agree to promptly sign any additional authorization forms that may be required by third parties in order for such results and records to be disclosed to the Company, and I understand that my refusal to sign such authorization forms will be grounds for rejection of my employment application or termination of my employment.
I understand that this employment application and any other Company documents are not contracts of employment and that should an offer of employment be extended, that employment will be "at will", for no specified duration and may be terminated by myself or the employer at any time, with or without cause. I also understand that no Company representative has the authority to enter into an employment contract, guarantee employment for a specified period, or modify any of the foregoing, other than in a written document signed by the President.
I certify that the information I provide on my employment application, in any accompanying documentation, in the course of the physical examination, and throughout the hiring process will be true, correct, accurate and complete. I understand and agree that any omissions, misrepresentations, or false or misleading information in this application, interviews or other aspects of the hiring process may disqualify me from further consideration for employment or, if already employed, such information may be grounds for the immediate termination of my employment, regardless of the timing or circumstances of discovery.
